The Aftercare Coordinator will facilitate access to a range of evidenced based treatments and after care services to clients who are completing or have completed a period of Residential Rehabilitation.
The Aftercare Coordinator will have extensive knowledge of relapse prevention strategies and will aim to facilitate sustained recovery and integration back into communities.
Aftercare services may include referral and / or management of co-morbid physical and mental health programs, support group meetings, counselling, advocacy and support.
The Aftercare Coordinator will possess high level knowledge of models of care for dealing with substance abuse victims, including the disease concept of alcoholism and will have knowledge of the social detriments of health and the cultural competence to address factors affecting the social and emotional well-being of Aboriginal people.
This role will involve travelling to Remote Communities in Central and Barkly Regions of the Northern Territory. It is expected that the Aftercare Coordinator will spend part of their time away from Tennant Creek.
BRADAAG will provide a vehicle for use, the Aftercare Coordinator will ensure that they comply at all times with the Vehicle Usage Policy.
Must have professional registration with AHPRA or AASW.
