The following forms are for corporations registered under the Corporations (Aboriginal and Torres Strait Islander) Act 2006 (CATSI Act).

• FORM: Notification of a change to corporation officers' details [8pp, PDF: 52Kb]

  Use this form for notifying ORIC of changes to the corporation officers’ details. This form must be lodged with ORIC within 28 days of the change.

• FORM: Request to change corporation name [2pp, PDF 39Kb]

  Use this form for notifying ORIC of a change to the corporation's name. This form must be lodged with ORIC within 28 days of the special resolution being passed.

• FORM: Request to change corporation rule book [2pp, PDF 39Kb]

  Use this form for notifying ORIC of a change to the corporation's rule book. This form must be lodged with ORIC within 28 days of the special resolution being passed.

• FORM: Notification of a change to corporation address and/or contact details [3pp, PDF 41Kb]

  Use this form for notifying ORIC of changes to the corporation's address and/or contact details.

• FORM: Application for reinstatement [4pp, PDF 49Kb]

  Use this form to apply to reinstate a corporation that has been deregistered.

• FORM: Voluntary deregistration [2pp, PDF 49Kb]

  Use this form to apply to deregister a corporation.

Annual reporting

2008–09: The reports required are different depending on a corporation's registered size and income. All corporations will need to complete a general report. ORIC sends all corporations a partly filled in copy of their general report. You can fill this in or use our new secure website to lodge online at https://online.oric.gov.au.
